Box 391 Redwood City, California 94064 CITY OF REDWOOD CITY ENGINEERING & CONSTRUCTION Attention: Mr. John Lynch PROPOSAL: QUALITY ASSURANCE INSPECTION SERVICES EL CAMINO REAL BEAUTIFICATION PROJECT REDWOOD CITY, CALIFORNIA Dear Mr. Lynch, At your request, Smith-Emery Company is pleased to submit this proposal to provide quality assurance inspection services for the E1 Camino Real Beautification project in Redwood City, Califomia. This proposal is based on the project schedule and information provided in our discussions with you regarding Smith-Emery Company's scope of work for the project. The scope of our services will include the following: quality assurance inspection services for construction activities by a resident field engineer/inspector who will track job production, contractor performance, materials, and progress payments. We understand the work will be performed during night shift hours over the entire course of the scheduled two and one-half month long project beginning in early to mid September, 2001. We anticipate providing one Field Engineer/Inspector to cover the scope of work outlined. Mr. Patrick Morrison will manage our work on the project and our proposed Field Engineer/Inspector will be Mr. Stuart Reeves, resumes attached. The Detailed Cost Estimate for the scope of work outlined above is attached. Any services requested that are not listed on the Detailed Cost Estimate will be billed in accordance with 'our standard Schedule of Fees, attached. Please note the cost for our services are dependent upon the schedule of the contractor and the estimated frequency of our inspections presented is subject to modification based on the construction schedule. ALL REPORTS ARE SUBMITTED AS THE CONFIDENTIAL PROPERTY OF CLIENTS.
