~t8.8 Maternity Leave of Absence Without Pay <br /> <br /> 18.8.1 Maternity leave of absence without pay or benefits shall be granted <br /> upon request to non-disabled probationary and permanent female <br /> Fire Rghters for that pedod of time requested, up to one (1) year. <br /> <br /> 18.8.2 Maternity leave shall be granted when the following conditions have <br /> been met: <br /> <br /> 18.8.2.1 The Fire Fighter shall notify her immediate <br /> supervisor in writing, accompanied by her physician's <br /> certificate of pregnancy, as soon as possible after <br /> pregnancy has definitely been determined, but no <br /> later than ninety (90) days prior to the tentative date <br /> on which the leave is to begin. Such notice shall <br /> include the tentative dates on which the leave shall <br /> begin and end. <br /> <br /> 18.8.2.2 Within thirty (30) days of the beginning of the <br /> matemity leave, the Fire Fighter shall submit to the <br /> Human Resources Director the specific date she <br /> intends to begin the leave, accompanied by her <br /> physician's written statement attesting to the Fire <br /> Fighter's ability to continue performing the full <br /> schedule of her duties and responsibilities. She shall <br /> continue on active duty until the specific date, <br /> providing she performs the full duties and <br /> responsibilities of her position and furnishes <br /> additional health statements form her physician upon <br /> reasonable request. <br /> <br /> 18.8.2.3 Prior to the establishment of a specific date for <br /> return to duty, the Fire Fighter shall submit to the <br /> Human Resources Director a notice of intention to <br /> return to duty, accompanied by her physician's <br /> statement certifying that the Fire Fighter is medically <br /> qualified to assume regular duties and <br /> responsibilities. <br /> <br /> 18.8.2.4 The Human Resources Director or his/her designee <br /> may designate the specific beginning and ending <br /> dates within the pay period requested by the Fire <br /> Fighter to meet the needs of the Fire Fighter and the <br /> c ty. <br /> <br /> 18.8.3 The Fire Fighter on leave shall be returned to her original position, <br /> or if that position is not in existence, to an equivalent position <br /> within her classification. <br /> <br /> 36 <br /> <br /> <br />
