EXHIBIT B <br /> <br /> Youth and Family Assistance <br /> Daybreak Shelter <br /> <br /> CONTRACT OBJECTIVES <br /> <br /> FISCAL YEAR 2001/2002 <br /> GOALS # OF PERSONS COMPLETION <br /> ASSISTED DATE <br /> <br /> Primary Quantitative Contract Goals <br /> <br />Twelve residents of Redwood City will be 12 persons June 30, 2002 <br />admitted to the YFA Daybreak Shelter <br />component. <br /> <br /> Other Secondary Collaborative Actions Outreach and Emphasis to be Conducted <br /> <br />Six residents of Redwood City will be 6 persons June 30, 2002 <br />admitted to Shared Housing or Aftercare <br />components. <br />Eighty percent of the youth with individual 10 persons June 30, 2002 <br />case plans will secure and maintain <br />employment. <br />Thirty percent of the youth with individual 4 persons June 30, 2002 <br />case plans will complete a job training <br />program or secure a high school <br />equivalency degree. <br />Sixty-five percent of the youth with 8 pemons June 30, 2002 <br />individual case plans will transition to a <br />positive living arrangement. <br /> <br /> <br />
