(OSSMC) <br />FISCAL YEAR 2007/2008 <br /> <br />CONTRACT OBJECTIVES <br /> <br />Quantifiable Goals to be Provided With <br />Re uested Funds <br />OSSMC will increase facility coverage in RWC by <br />5% over the prior year <br /> <br />We will investigate 85 cases involving individuals in <br />the City of Redwood City, to verify 75% of those <br />cases and to bring satisfactory resolution to 70% <br /> <br />Performance Indicators (how you will measure <br />the uantifiable oal <br />Ombudsmen file a monthly activity report with the <br />office, including facility visits. This data is entered <br />into a data base. Quarterly the data is analyzed to <br />determine if we have met the minimum <br />recommended visits as per our facility coverage <br />Ian. <br />Data gathered in the investigation process is <br />compiled in a case report and entered into a <br />database. This data is analyzed regularly and <br />provides us with statistics on ethnicity, age and <br />gender of the residents, types of complaints, <br />verification and resolution information. Statistics are <br />sent to the State and from there to the National <br />Ombudsman Reporting Center in Washington for <br />further analysis. The detailed data gathered by the <br />local programs has been praised by other Older <br />Americans Act Programs and will be used as a <br />model for those ro rams to ather their data. <br /> <br /># of Unduplicated RWC Residents/Households to be Assisted (Only programs serving individuals <br />ma re ort b individuals.
