6.0. - Page 134 of 179 <br />SP -60 Homelessness Strategy — 91.215(d) <br />Reaching out to homeless persons (especially unsheltered persons) and assessing their <br />individual needs <br />The City funds LifeMoves and they provide shelter and supportive services. In addition they provide <br />outreach to unsheltered persons to assist them in obtaining shelter or other services to help them end <br />homelessness. <br />Addressing the emergency and transitional housing needs of homeless persons <br />The City will fund several shelters that also provide support services for homeless individuals, families, <br />youth, victims of domestic violence and those with special needs (mental illness). Support services <br />include employment assistance, counseling, health care, and housing referrals. <br />Helping homeless persons (especially chronically homeless individuals and families, families <br />with children, veterans and their families, and unaccompanied youth) make the transition to <br />permanent housing and independent living, including shortening the period of time that <br />individuals and families experience homelessness, facilitating access for homeless individuals <br />and families to affordable housing units, and preventing individuals and families who were <br />recently homeless from becoming homeless again. <br />The City's core service center at the Fair Oaks Community Center is the primary intake and referral <br />location for all types of human services and homeless needs. Service providers at the center provide <br />basic safety net services. <br />Help low-income individuals and families avoid becoming homeless, especially extremely <br />low-income individuals and families who are likely to become homeless after being <br />discharged from a publicly funded institution or system of care, or who are receiving <br />assistance from public and private agencies that address housing, health, social services, <br />employment, education or youth needs <br />In addition to the basic safety net services provided at the Fair Oaks Community Center the City funds <br />programs that provide housing referrals/matching, rape trauma services for children and Ombudsman <br />services for frail elderly and disabled persons in community care facilities. <br />Consolidated Plan REDWOOD CITY 128 <br />OMB Control No: 2506-0117 (exp. 06/30/2018) <br />E'1 <br />
