EXHIBIT A <br />Scope of Work <br />Contractor will clean and dispose of materials contaminated by rodents and install new rodent <br />prevention measures for future facility protection. Contractor's services will include: <br />A. Covering work area <br />B. Removal of contaminated insulation: <br />a. Removal of all insulation stained, contaminated, or affected by rodents. <br />C. Disposal of contaminated insulation: <br />a. Hauling off and disposing of all removed contaminated insulation at <br />designated approved fiberglass dump sites. No trash or insulation shall be <br />left on site. Insulation will be disposed of at the end of each work day. <br />D. Cleanup: <br />a. Vacuum out all rodent evidence (droppings, carcasses, etc.) using light <br />vacuum machines to avoid noise disturbance. <br />E. Sanitization: <br />a. Spraying a DSV (Disinfecting, Sanitizer, Virucide) solution using Ultra - <br />Low -Volume Fogger. <br />F. Insulation installation: <br />a. R19 Eco Batts insulation manufactured by Knauf. <br />G. Rodent prevention: <br />a. Seal and block all entry points using foam sealer, caulking, adhesive and <br />replacing mesh wire and/or vents as needed. <br />b. Setting bait, trap & kill station: one extermination unit every 300 sq ft. <br />H. Monitoring & containment of rodent activity: <br />a. Surveillance of rodent activity, infestation, contamination and damage <br />b. Removal and disposal of carcasses. <br />c. Cleaning and sanitizing of heavily affected areas. <br />d. Ongoing rodent prevention workâsealing of new entry points. <br />e. 2 visits in the first month following completion of work <br />f. 1 visit every month for 12 months <br />g. 1 visit every 6 months for 36 months. <br />Contractor will complete the tasks listed in subsections A through G above within 90 days of the <br />Effective Date. <br />2. Payment Terms <br />For all the work outlined above, the City will pay Contractor a not to exceed amount of $28,000. <br />City will pay Contractor a flat fee of $23,000 following satisfactory completion of the tasks <br />listed in subsections A through G of the Scope of Work within 90 days of the Effective Date. <br />City will pay Contractor a not to exceed amount of $5,000 for monitoring and containment <br />services throughout the ensuing 36 -month period; payment for monitoring and containment <br />services shall be invoiced individually after completion of each follow-up visit. <br />
