❑ If appropriate, inform the Policy Group and issue an Emergency Proclamation, and coordinate <br />local government proclamations with other emergency response agencies <br />Shift Change: <br />❑ Provide turnover briefing to position replacement <br />❑ Provide details regarding ongoing activities and planned activities to be accomplished during the <br />upcoming operational period. <br />•FORM: ICS -214 <br />❑ Ensure next shifts staff are accounted for. Ensure the safety and well-being of staff being <br />dismissed for the operational period <br />❑ Provide all completed documentation to the Documentation Unit <br />❑ Follow EOC check out procedures, including signing out <br />• FORM: ICS -211 <br />Demobilization and Termination: <br />❑ Authorize demobilization of sections, branches and units when they are no longer required <br />informally or via a plan <br />• PLAN: DEMOBILIZATION PLAN <br />❑ Notify higher level EOCs and other appropriate organizations of planned demobilization, as <br />appropriate <br />❑ Ensure that open actions not completed will be handled after demobilization <br />❑ Ensure that all required forms or reports are completed prior to demobilization <br />❑ Prepare to provide input to the after -action report <br />❑ Proclaim termination of the emergency response and proceed with recovery operations <br />❑ Alongside Public Information Officer, make emergency termination notifications to City Council, <br />Response Partners, Community and Operational Area <br />❑ Provides a final emergency briefing of the event to EOC personnel to include: <br />• Date/time of termination <br />• Requests all documentation developed during the event response <br />• Instructions for support of recovery operations or assembly of the final report <br />• Time and date of formal debrief to identify issues, lessons learned and corrective actions <br />• Instructions for resumption of normal operations <br />❑ Determine if a formal Recovery Plan is required based on a review of the technical criteria by: <br />• Length and resources required for investigating and fact-finding activities <br />• Assessment of property damage efforts require substantial and prolonged coordination and <br />communications with off-site governments, agencies and/or response organizations <br />• Number of personal injuries or illnesses requiring protracted follow-up treatment, analysis <br />and public information <br />11 Supervise the transition of the EOC from response to recovery operations, as necessary <br />
