Activation: <br />Check-in at the EOC <br />• FORM: ICS -211 <br />• FORM: ICS -205 <br />❑ Assist with EOC Set-up (if not already fully set-up) <br />❑ Check work station to ensure readiness <br />❑ Wear identification vest and read over job aid <br />❑ Receive situation, section and position briefing from available and appropriate personnel <br />❑ Ensure readiness to maintain concise records of position activities <br />• FORM: ICS -214 <br />❑ Based on the situation, activate the necessary Units within the Fire & Rescue Branch <br />❑ If the mutual aid system is activated, coordinate use of Hazmat resources with the Operational <br />Area Mutual Aid Coordinator <br />• FORM: HAZMAT DEPLOYED RESOURCES <br />Response: <br />Obtain and maintain current status on hazardous material missions being conducted in the City <br />❑ Request mutual aid resources through proper channels and notify the Fire and Rescue Branch <br />Director <br />❑ Ensure that the appropriate agencies are notified through the Incident Commander: <br />• Local Administering Agency <br />• State Warning Center <br />• National Response Center <br />• Other local, state and federal agencies as needed <br />❑ Evaluate and recommend public protection action options to the Fire and Rescue Branch <br />Director <br />Refer all contacts with the media to the Public Information Branch <br />Ensure that Unit position logs and other necessary files are maintained <br />Shift Change: <br />Provide turnover briefing to position replacement <br />❑ Provide details regarding ongoing activities and planned activities to be accomplished during the <br />upcoming operational period <br />• POSITION JOB AID <br />• FORM: CURRENT ICS -214 <br />❑ Provide all completed documentation to the Documentation Unit <br />C-40 <br />
