CONSTRUCTION PERMIT <br />ENGINEERING & CONSTRUCTION <br />CITY OF REDWOOD CITY, CALIFORNIA <br />www.redwoodcity.org <br />PERMIT NO. <br />The City of Redwood City ( "City ") hereby grants this Construction Permit ( "Permit ") to the City and <br />County of San Francisco, a municipal corporation ( "Permittee "), for the purpose of performing <br />improvements at the following location(s) within City's boundaries: <br />. This Permit is issued in accordance with <br />applicable law and the Memorandum of Agreement ( "MOA "), dated , 2009, by and <br />between Permittee and City. The parties' respective insurance and indemnity obligations in connection <br />with the construction work authorized by this Permit shall be as stated in the MOA. <br />This Permit is granted subject to the following conditions: <br />(1) The amount of fees and deposits payable by Permittee pursuant to this Permit shall be as <br />stated in the MOA. <br />(2) Reference is hereby made to the attached application for permit executed by Permittee <br />under date of and the same is hereby incorporated herein and made a part hereof as set <br />forth herein verbatim. <br />(3) Permittee will be responsible for Project safety and shall erect barriers or warning devices <br />as required by the WATCH hand book (Work Area Traffic Control Handbook by BNi and endorsed by <br />APWA) and Permittee's approved traffic control plans. <br />(4) All work to be done by Permittee shall be performed in accordance with and subject to all <br />terms and conditions of the specifications and plans referred to in the MOA, and to the "Engineering <br />Standards" specified in the MOA, approved by the City Engineer, and by this reference incorporated <br />herein and made a part hereof. <br />(5) Permittee is responsible to make arrangements with its design engineer for the <br />preparation of the "as- built" plans. The "as- built" plans shall be the original tracings or permanent mylar, <br />good quality, showing the engineer's certificate as stated above. <br />(6) The grant of this Permit shall be for a term of two years from the date of issuance but <br />may be suspended by written notice from the City Engineer during the pendency of any material violation <br />of applicable City ordinances that imperils the safety of City's facilities or populace. <br />(7) Conform to Construction Best Management Practices (BMP), as detailed in the Attached <br />Booklet entitled "Blueprint for A Clean Bay ". <br />(8) Other: -At least 24 hours prior to any required inspection, Permittee shall contact City <br />Inspector Terrence Lewis at 650- 740 -7367. <br />GRANTED: CITY ENGINEER <br />CITY OF REDWOOD CITY <br />Date: By: <br />D -1 -3 <br />City of Redwood City MOA ver 030910 <br />
