01/10/2011 <br /> category of coverage as nearly comparable as possible to the benefits provided <br /> under the Group Health Benefits Plans provided by the City, according to the <br /> plan selected by individual employees. The City need only expend for this <br /> purpose the actual amount required to achieve parity between the benefits <br /> agreed to be provided under the Group Health Benefits Plans provided by the <br /> City and the benefits provided under any federal or state plan as supplemented <br /> in the manner hereinabove described. In no event shall the City be required to <br /> expend for such purposes an amount which, when added to the contributions <br /> or taxes required of the City under the federal or state act, shall exceed the <br /> amounts paid at the time such legislation becomes effective. <br /> If the benefits provided under the federal or state act exceed the benefits <br /> provided hereunder in each category of coverage, the City shall be under no <br /> further obligation to make any contributions. <br /> 19.4 Life Insurance <br /> The City shall continue to offer to eligible employees additional life insurance <br /> equal to one and one -half (11/2) times the employee's annual salary, at a 60/40 <br /> premium contribution split between the City and the employee respectively. <br /> 19.5 Vision Care <br /> Effective the 1s of the month directly following ratification and Council approval <br /> of agreement, the City shall contract with Vision Service Plan (VSP) or a <br /> comparable vision care provider to provide Vision Care Plan B benefits for <br /> LA.F.F. employees and their dependents. The Vision Service Plan B provides <br /> for an exam every 12 months, lenses every 12 months if needed, and frames <br /> every 2 years if needed. There is to be no deductible for employees, but a <br /> $20.00 per person deductible will apply to dependents each time benefits are <br /> available and will be paid by the employee. <br /> 19.6 Long Term Disability Insurance <br /> The City will reimburse twenty -five dollars ($25.00) per month per employee <br /> towards the cost of Long Term Disability. <br /> SECTION 20 - SAFETY <br /> 20.1 Both the City and the Union shall expend every effort to see to it that work is <br /> performed with a maximum degree of safety, consistent with the requirement <br /> to conduct efficient operations. <br /> 20.2 As required by the National Fire Protection Association (N.F.P.A.) sections 1500; <br /> 5 -3,9; and 1404; 4 -2,3, and the Code of Federal Regulations (CFR) /title 8/5144, <br /> the employer shall not permit respirators with tight fitting face pieces to be <br /> worn by employees who have facial hair that comes between the sealing <br /> 46 <br /> Page 52 #15077 <br /> MUFF # 905 <br />
