Please submit with your application the following <br />information to verify your eligibility based on income: <br />- Income documents: Federal Form 1040, copies of previous 3 months check <br />stubs, copies of Social Security, SSI, or AFDC award letters, etc., used to <br />determine income eligibility. <br />- Utility payments: Copies of water & sewer and gas and electrical bills paid <br />during the previous 6 - 12 months, to be used to determine average utility payment <br />costs. <br />- Replacement Unit verification: If you have located a replacement unit and have <br />been accepted, please attach a copy of your new rental lease to the application. <br />Upon receipt of your Relocation Needs Assessment Form Application and the required <br />attachments, you will be contacted to attend a needs assessment interview to finalize your <br />Relocation Plan and arrange procedures for you to receive your relocation benefits. <br />If you have not yet begun to look for a replacement unit, you must do so to assure that <br />you can move no later than in order to qualify for the relocation benefits.
