Red — wood` <br />Founde <br />rauntletl 1881 <br />Council Benefits Summary <br />Public Employees Retirement System (PERS) — Council Members shall be entitled to participate <br />in the PERS Retirement System. The City will contribute both the employee and the employer's <br />portion. <br />Health Insurance — Council Members shall be entitled to health insurance provided by the City. <br />The plans offered shall be those of the California Public Employees Retirement System. <br />Flexible Benefit Plan - City Council Members shall be entitled to participate in the Flexible Benefit <br />program. The City shall contribute amount equal to the Kaiser Family Rate to establish the Flexible <br />Benefit Plan for the purpose of payment for PERS Medical Premiums, Dependent Care, Health <br />Care Reimbursement and the Management Cash Option (as described below). <br />♦ If the City Council Member elects no health plan coverage as a health savings/cash option the <br />City shall share equally the amount designated as the City's actual maximum monthly <br />contribution. <br />♦ If the City Council Member elects coverage in health, medical spending and /or dependent care <br />the City and the Council Member will share equally in the difference between the elected <br />coverage and the monthly contribution to a maximum of $102.50 <br />Dental Insurance — City Council Members shall be entitled to participate in dental insurance <br />provided by the City with an annual cap of $2,000 for basic coverage and $2,500 lifetime cap for <br />orthodontics coverage. <br />Life Insurance — City Council Members shall be entitled to Life Insurance with limited principal <br />amount of $3,000.00 <br />Vision Care Insurance — City Council Members shall be entitled to the Vision Care Service Plan <br />for self and dependents. <br />Long Term Disability — City Council Members shall be entitled to long term disability coverage. <br />The city will pay the full cost of the basic rate. <br />Mileage Reimbursement — Council Members shall be reimbursed at the prevailing IRS rate for <br />travel requested and authorized by the City. <br />1 4 40 3 <br />
