If you have any questions, you must call them at 916- <br />322-5662 for legal advice, as we are just the designated filing office for them. The 1994 <br />Form 490 is the current form they are using for distribution to you at this time, just be <br />sure to use the correct current dates on each appropriate page. <br /> <br />If you need any other FPPC Forms, please call me Mon-Thurs. 780-7222. <br /> <br /> <br />
