MEMORANDUM <br /> <br />TO: HONORABLE MAYOR AND COUNCILMEMBERS <br /> <br />FROM: CITY CLERK <br /> <br />DATE: JUNE 17, 1993 <br /> <br />SUBJECT: OFFICEHOLDER OR CAMPAIGN COMMITTEE FILING REQUIREMENTS <br /> <br />Enclosed is Form 490 for you to complete, sign, date and file in this office <br />by July 30, 1993. This form covers the dates of November 3, 1992 through <br />June 30, lg93 if you were elected in November, or January 1, 1993 through <br />June 30, 1993 if you were an incumbent at the time of the November election. <br /> <br />You must file a semi-annual Form 490 as long as you have a campaign committee <br />or office holder committee. If you wish to terminate your committee you must <br />disburse your funds to acceptable non-profit organizations or campaign <br />expenses, file a Form 490 showing a zero balance, file a Form 415 termination <br />form with the Secretary of State, and close your bank account and file a <br />Form $02. <br /> <br />Be sure and file the enclosed Form 490 in this office by July 30~ lg93. <br /> <br /> <br />
