Officeholders who have a <br />committee or intend to spend or raise more than $1,000 per calendar <br />year, must file a form 490. If you wish to terminate your committee <br />you must file a Form 415 with the Secretary of State; a Form 490 <br />showing a zero balance, with me; and file Forms 501 and 502 with the <br />FPPC. <br /> <br /> <br />
