EXHIBIT "A" <br />SCOPE OF SERVICES AND FEE <br />For Door #: 2-1, 2-31 3-3, 3-4, 3-6, 4-1, 44, 4-5 and 4-7 <br />Contractor shall install nine GH7521 LS Liftmaster operators with: <br />1. 230 Volts, 3/4 HP, 1 Phase motors. <br />2. Nine chain hoists for manual operation. <br />3. Nine- motor brackets for head plate mounting. <br />4. Nine - 3 button control stations. <br />5. Installation of photo eyes for auto reversing devices. <br />6. Nine - Miller safety edges with two wire coil cords. <br />7. Contractor shall remove and dispose of damaged operators. <br />Cost: $25,668.00 <br />For Door # 3-1, 3-2, 3-14, 3-15 and 4-8 <br />Contractor shall repair five Power Master commercial roll up door operators by: <br />1. Replacing damaged/worn 51-290 V -Belt. <br />2. Aligning and tightening drive chains. <br />3. Setting limits as needed. <br />4. Servicing and lubricating. <br />5. Removing and disposing of damaged parts. <br />Cost: $795.00 <br />For Door # 4-6 and 4-8 <br />Contractor shall install Liftmaster photo eyes for safety device on two doors including: <br />1. Running new control wire to each side of the door for photo eyes. <br />2. Mounting eyes 6" off the floor. <br />3. Adjusting limit switches as needed. <br />4.
