CALIFORNIA DEPARTMENT OF FISH AND WILDLIFE <br />BAY DELTA REGION <br />7329 SILVERADo TRAIL <br />NAPA, CALIFORNIA 94558 <br />(707) 944-5500 <br />WWW .W ILDLI FE.CA.GOV <br />STREAMSED ALTERATION AGREEMENT <br />NOTIFICATION No. 1600-2013-0356-R3 <br />Atherton Creek <br />CITY OF MENLO PARK <br />ATHERTON CHANNEL FLOOD CONTROL MAINTENANCE <br />Exhibit A-1 <br />This Streambed Alteration Agreement (Agreement) is entered into between the <br />California Department of Fish and Wildlife (CDFW) and the City of Menlo Park <br />(Permittee) as represented by Azalea Mitch. <br />RECITALS <br />WHEREAS, pursuant to Fish and Game Code (FGC) section 1602, Permittee notified <br />CDFW on September 6, 2013 that Permittee intends to complete the project described <br />herein. <br />WHEREAS, pursuant to FGC section 1603, CDFW has determined that the project <br />could substantially adversely affect existing fish or wildlife resources and has included <br />measures in the Agreement necessary to protect those resources. <br />WHEREAS, Permittee has reviewed the Agreement and accepts its terms and <br />conditions, including the measures to protect fish and wildlife resources. <br />NOW THEREFORE, Permittee agrees to complete the project in accordance with the <br />Agreement <br />PROJECT LOCATION <br />This Agreement authorizes routine maintenance in areas of Atherton Creek that fall <br />under the jurisdiction and responsibility of the Permittee. Atherton Creek is tributary to <br />Bayfront Canal which flows into Flood Slough and ultimately into San Francisco Bay; in <br />Redwood City, State of California. <br />Project locations include: <br />Site 1. Atherton Creek between Highway 101 and Haven Avenue; Latitude <br />37029'06.83"N, Longitude 122°10'54.50"W; adjacent to Assessor's Parcel <br />Number (APN): 055-231-060 and including APN 055-232-110. <br />Ver 0),16,2010 <br />
