Exhibit A-1 <br />Notification #1600-2013-0356-R3 <br />Streambed Alteration Agreement <br />Page 15 of 23 <br />• All plantings shall have a minimum of 80% survival at the end of 8 <br />years. <br />• Vegetation cover shall consist of no more than 10% non-native <br />species. <br />• If the survival and/or cover requirements are not meeting these <br />goals, Permittee is responsible for replacement planting, additional <br />watering, weeding, invasive exotic eradication, or any other <br />practice, to achieve these requirements. Replacement plants shall <br />be monitored with the same survival and growth requirements for <br />five years after planting. <br />Re -vegetation monitoring shall be conducted annually for a period of <br />eight (8) years to determine whether these goals have been met. If <br />the survival and/or cover requirements are not projected to meet <br />these goals, based on annual monitoring, Permittee is responsible for <br />replacement planting, additional watering, weeding, invasive exotic <br />eradication, or any other practice(s) that would to achieve these <br />requirements. <br />3. Reporting Measures <br />Permittee shall meet each reporting requirement described below. <br />3.1 Notification of Proposed Activities. Permittee shall provide CDFW <br />written notification of proposed routine maintenance activities to be <br />performed in the upcoming year by March 15 each year. Notification <br />reports shall describe the project location, general topography, <br />hydrological features, vegetative cover within 50 feet of the work <br />area, length and width of impact area, cubic yards of sediment to be <br />removed and a detailed description of proposed modifications to the <br />banks and/or channel. Reports shall be submitted to CDFW <br />regardless of whether work is proposed. <br />CDFW shall append annual notification reports of proposed <br />maintenance activities to this Agreement. For streamlined tracking, <br />Permittee shall label annual notification reports according to the <br />following convention: Exhibit C -[year] (e.g. Exhibit C-2015, Exhibit C- <br />2016). <br />3.2 Annual Reports for Completed Projects. On an annual basis, <br />Permittee shall provide CDFW written notification of maintenance <br />projects completed. Annual reports shall include the project <br />identification (site name and location), a brief project description, and <br />the appropriate fee from the current CDFW Streambed Alteration <br />
