6.E. - Page 30 of 44 ATTACHMENT A- PROPOSAL PRICING FORM Price must be complete and include tax, delivery, labor, and other charges. Prices remain firm for one- year after award of contract to Successful Proposer. If the unit price and the total amount named by a Proposer for any item do not agree, it will be assumed that the error was made in computing the total amount and the unit price alone will be considered as representing the Proposer's intention. All proposal items, including lump sums, unit prices, and additive alternates, must be filled in completely. Quote in numerals only, unless words are specifically requested: Proposers should ensure that the following minimum information is included in Chapter 6 — Proposal Cost. It is expected that material, hardware, and software costs will be passed onto the City at the purchase price, with a copy of the purchase receipt for reimbursement. In the event the Proposer charges an additional fee or markup cost on materials, hardware or software such charges shall be listed in chapter 6. Any additional costs not specified in this attachment should be identified in chapter 6; the City will not honor any additional charges that are not specified in chapter 6. Proposers should include the following minimum information in chapter 6 of the proposal: ■ Proposer' s hourly labor rate schedule (specify hourly rate for each position); ■ Mileage rate; ■ Travel time and rate; ■ If there is a minimum charge per service call; ■ Additional/ mark- up fees on materials, hardware, or software; ■ If there are any surcharges for emergency calls, second trips, or weekend/ after hour calls.
