i. A description of the type of service or work requested; <br />ii. A start date and end date of the assignment; <br />iii. Work location; <br />iv. Work schedule; <br />v. Any certifications that may be required; and <br />vi. Level of expertise and/or education required. <br />A copy of the request will be provided to the City's Human Resources Director. <br />c. The Contractor will respond to the Director's request within five (5) business days <br />with an acknowledgement of the request and a timeline for providing personnel for <br />consideration for an assignment. The timeline shall not exceed fifteen (15) business <br />days from the date of the Director's request. <br />d. The Contractor shall introduce each candidate to be considered for an assignment <br />in a separate email containing, at minimum: <br />i. The candidate's resume; and <br />ii. The candidate's hourly billing rate. <br />e. The Director may request to meet with one or more candidates <br />f. The Director shall notify the Contractor via email who the preferred candidate shall <br />be to fulfill the assignment, and issue a task order in substantially the same form as <br />attached Exhibit A-1. <br />g. Contractor shall furnish City with every reasonable opportunity to ascertain that the <br />services of Contractor and Assigned Personnel are being performed in accordance <br />with the requirements and intentions of this Agreement. All work done and all <br />materials furnished, if any, shall be subject to the Director or his/her designee's <br />inspection and approval. The inspection of such work shall not relieve Contractor <br />of any of its obligations to fulfill the Agreement as prescribed. <br />h. In the event that any Assigned Personnel become unavailable due to resignation, <br />sickness, or other factors, Contractor is responsible for timely provision of qualified <br />replacements. The Director or his/her designee shall have sole discretion whether <br />to accept Contractor's personnel proposed for assignment. <br />In the event that Contractor, at any time during the term of this Agreement, desires <br />to reassign, substitute or remove Assigned Personnel, Contractor shall notify the <br />Director five (5) business days before the day of reassignment, substitution or <br />removal. The Director or his/her designee shall have sole discretion whether to <br />accept Contractor's personnel proposed for reassignment or substitution. <br />j. If disclosure under the Political Reform Act and the City's Conflict of Interest Code <br />is required of Contractor or any of Contractor's employees, Contractor or <br />Contractor's affected employees shall complete and file with the City Clerk those <br />schedules specified by City and contained in the Statement of Economic Interests <br />Form 700. <br />REV: 10-04-19 EI <br />Contractor, for Contractor and on behalf of Contractor's employees, warrants that <br />by execution of this Agreement, that they have no interest, present or contemplated, <br />ATTY/AGR.2019.258/HB Staffing Services (Page 7 of 14) <br />
