EXHIBIT "A" <br />SCOPE OF SERVICES AND FEE <br />Concrete Walkway Repairs: <br />1,300 Square Feet. Saw -cut, demolish, and remove areas of failed concrete at Red <br />Morton Park, as identified in walk through with Parks staff. Excavate and off haul debris. <br />Scarify and compact subbase. Form, set reinforcing steel, and install expansion joints. <br />Pour and finish 4" concrete walkways with standard finish and deep score pattern to <br />match existing. Save and reset medallions where applicable. <br />Cost: $24,505 <br />II. Remove and Replace Tree Planter Curbwall: <br />Saw -cut, demolish, and remove the existing failed concrete tree planter curb. Excavate <br />and off haul debris. Set forms and reinforcing steel with expansion joints for new larger <br />planter curb. Pour and finish new curb wall to match existing pattern with standard finish. <br />Cost: $9,950 <br />III. Added 244 SF Walkway Repairs: <br />Saw -cut, demolish, and removed added 244 SF of existing concrete. Excavate and off <br />haul debris. Scarify and compact subbase. Form, set reinforcing steel, and install <br />expansion joints. Pour and finish 4" concrete walkways with standard finish. <br />Cost: $7,500 <br />IV. New ADA Ramp: <br />Saw -cut, demolish, and remove one location of concrete for new ramp. Excavate and oft <br />haul debris. Set forms and truncated dome pad with #4 reinforcing steel and expansion <br />joints. Pour and finish new 4" concrete ADA ramp. <br />Cost: $4,500 <br />TOTAL: $46,455 <br />Contractorwill be paid in one installment, upon completion of the Services and City receipt <br />of an invoice. <br />RN: 03-05-2020 RL <br />
