Each <br />attachment must be identified by Attachment Number And Title in the upper dellt hand corner of the page. If the attachment <br />involves multiple pages, you must put the Attachment Number And Title on a separate divider page in front of the actual attachment <br />pages. If a particular Attachment is not available, not applicable, or the infonnation is contained in one of the other Attachments, you <br />still must provide a separate place-holder page with the Attachment Number and Title and indicate the status ofthat Attachment. All <br />Attachments must be placed behind the application in numerical order. The entire application should be clipped together; do not use <br />staples. We suggest that you print out the entire application betore you begin. Navigating through the <br />application is a simple process, When you open the document, your cursor will be located in the first grey highlighted, fill-in box; use <br />the "Tab" key to move from box to box. Using "Shift-Tab" moves you backwards through the various responses. To mark a check <br />box, tab to the appropriate box and click on it; this will mark it with an X; clicking the same box again removes the X. Your answers <br />will show in grey highlight; this should disappear when you actually print the document. Except for the check boxes, you should put <br />an entry into each box; if the question does not apply to your program/project put "n/a". Starting on Page 2 are a series of questions; <br />tab to the answer box which will expand to accommodate your answer; we encourage you to be concise, but provide enough detail for <br />us to adequately evaluate your application. Note: you will not be able to use the tab function when typing your answers because that <br />function is superceded to navigate you to the next question.
