EXHIBIT " A" <br />SCOPE OF SERVICES AND FEE <br />The services to be provided under this Agreement consist of outreach and engagement by two <br />full-time (2.0 FTE) Case Managers to unsheltered individuals and families experiencing <br />homelessness in the City of Redwood City [and the surrounding unincorporated areas, including <br />North Fair Oaks]. Specifically, services will include: <br />• Conducting outreach and engagement services to unsheltered individuals experiencing <br />homelessness across the service area, and in particular, individuals living in RVs parked on the <br />streets in the City; <br />• Working with designated officials from the City of Redwood City or other agencies to identify <br />where homeless individuals reside, and responding to those concerns; <br />• Responding promptly to calls from the Redwood City Police Department regarding issues involving <br />unsheltered homeless individuals in the service area. <br />• Connecting clients to available community services, including to primary and behavioral health <br />care providers, housing resources, and other providers of basic needs (such as meals, showers, <br />toiletries, and emergency shelter). <br />• Working with City officials to start up and launch a Safe Parking program for those living in RVs <br />and other residents of Redwood City; <br />• Providing intensive case management services to approximately 40 individuals each year, <br />including connecting individuals to San Mateo County's Coordinated Entry System (CES) in order <br />to access available shelter beds; <br />• Participating in Multi -Disciplinary Team (MDT) meetings to strengthen the collaborative effort to <br />effectively provide services to the unsheltered population in the region. The MDT utilizes a classic <br />case management model, which is effective because it brings together a unique set of community <br />members, all invested in each homeless individual's success. <br />Objectives: During the grant period, LifeMoves will achieve the following objectives: <br />• LifeMoves will initiate contact with 90% of homeless individuals identified by Redwood City <br />officials; <br />• 90% of homeless individuals contacted by Homeless Outreach Team (HOT) will be referred either <br />to the City's Safe Parking program, or to CES for connection to available shelter beds and other <br />services; and <br />70% of homeless individuals contacted by HOT will be offered ongoing, comprehensive case <br />management, subject to caseload availability. <br />ATTY/AGR.2020.092/LifeMoves (Page 11 of 12) <br />
