6.C. - Page 27 of 28 <br />Exhibit "A" to MLA <br />Form of Site License Authorization <br />This Site License Authorization ("SLA") is approved by the city of Redwood City <br />("City") this day of , 20 (the date executed by all parties, referred <br />herein as "SLA Effective Date"). <br />This SLA, dated as of the date set forth below, is entered into by and between the City and <br />'a ("Licensee") and is pursuant to that certain <br />Municipal Facilities Master License Agreement dated , 20_ (the "MLA"). <br />1. SLA. Licensee has submitted a request for an encroachment permit in conjunction <br />with this application for an SLA pursuant to the MLA. City has reviewed the <br />application to use Municipal Facility and grants approval subject to the terms of this <br />SLA. All of the terms and conditions of the MLA are incorporated hereby by <br />reference and made a part hereof without the necessity of repeating or attaching the <br />MLA. In the event of a contradiction, modification, or inconsistency between the <br />terms of the MLA and this SLA, the terms of this SLA shall govern. Capitalized <br />terms used in this SLA shall have the same meaning described for them in the MLA <br />unless otherwise indicated herein. <br />2. Site Name/No.: <br />3. Site Address: <br />4. This SLA is not a building permit or other regulatory approval to install any <br />Equipment (as defined in the MLA). <br />5. Equipment and Locations. Licensee shall have the right to attach the Equipment <br />described in Attachment 1 to the specific space on the specific Municipal Facility <br />identified in Attachment 2. <br />6. Term. The term of this SLA shall be as set forth in Section 2 of the MLA. <br />7. Reserved. <br />8. Installation Date. The earlier of ninety (90) days from full execution of an SLA, or <br />the date that any Equipment is first installed by Licensee pursuant to an SLA issued <br />by the City in accordance with the MLA. If the latter, Licensee shall document in <br />writing the date by notice to the City in accordance with Section 1.4 of the MLA. <br />9. Termination. Licensee upon tendering one hundred and eighty (180) days written <br />notice to the other party may terminate this SLA. City may terminate this SLA as set <br />forth in MLA Section 11 (Termination for Default).
