<br />6.1C <br />Page 10 <br /> <br />Exhibit 01 <br />Monitoring <br /> <br />Early Entry Shelter Program <br />FY 2008-09 <br /> <br />No request for reimbursement will be honored until all reports required hereunder have been <br />received by the Department of Housing. <br /> <br />Contractor shall submit to the Human Services Agency the following reports within 30 days of <br />the end of each quarter: <br /> <br />1. Performance Log (optional), to be submitted on form provided by the County Human <br />Services Agency (HSA), which shall provide income and demographic information of each <br />individual or household served as well as a brief description of the services provided during <br />the quarter. <br /> <br />2. Performance Summary, to be submitted on form provided by the County HSA, which shall <br />sununarize the number of clients served by their ethnicity and income. <br /> <br />3. Consolidated Funding Source Reoort. <br /> <br />4. Quarterlv Proiect Narrative Report. which provides a narrative ofhigblights/details of both <br />the funded activity and of the Contractor's organization. <br /> <br />Contractor is receiving CDBG funding and agrees to complete and submit HUn required <br />quarterly reports as stated in Exhibit D to ashirkhani@smchsa.org, electronically on the quarterly <br />basis. <br /> <br />Perfonnance reporting fonns will be provided to Contractor by the County of San Mateo Human <br />Services Agency Electronically. <br /> <br />An evaluation ofEESP participant outcomes will be prepared at the end of the year. <br />
