Conduct case management sessions 16 June 30, 2009 <br /> with adult clients and assist them in <br /> setting and achieving self-sufficiency <br /> goals. 80% of clients who complete the <br /> Emergency Shelter program achieve at <br /> least 60% of their Care Plan goals. <br />. 90% of adult clients will exit to 18 <br /> permanent or transitional housing <br /> June 30, 2009 <br /> <br />1 Primary quantitative contract goal is the number of households/persons you will assist under <br />this contract as stated in your grant application. Contract performance will be measured by this <br />quantitative goal. <br /> <br />2 Other secondary goals are the efforts you will take to deliver your services for which <br />performance will be discussed in your annual narrative. <br />
