<br />Attachment A <br /> <br />o Ability to successfully work with high needs students <br />including special education students <br /> <br />Evaluation <br />o Participate in the District's after school evaluation which may include <br />the following: <br />o Administration of pre and post student surveys <br />o Provide a list of program attendees including names, student <br />IDs, student program start and end dates, and the total <br />number of days attended <br />o Provide access to program participants for focus groups <br />annually <br />o Share any evaluation data that you collect to be included in <br />the District-wide evaluation <br /> <br />Fiscal/ Administrative <br />o Track attendance daily and submit bi-annual attendance reports with <br />invoices reflecting the earned attendance reimbursement for that <br />period. <br /> <br />Partnersh ip /Comm u nication <br />o Participate in quarterly collaborative after school provider meetings <br />organized by the District <br />o Work in partnership with site Principal and/or Community School <br />Coordinator to develop, implement and evaluate program <br />o Site Coordinator checks in weekly with Principal or Community School <br />Coordinator either in person or byemail <br />o Site Coordinator provides monthly updated participant lists to <br />Principal or Community School Coordinator <br />o There is a structured system in place to share information about <br />specific students and their needs, challenges and accomplishments <br />o A referral process is in place that identifies students most in need and <br />most appropriate for the program <br />o Work in partnership with school to develop plans to modify <br />programming as needed based on annual evaluation data <br /> <br />School Expectations: <br />o Provide student id numbers to provider for all students enrolled in the <br />prog ram <br />o Make time available at appropriate meetings (staff, school site council, <br />etc) to inform school of the program and facilitate communication <br />o Make adequate space available for a fulltime (a minimum of 30 hours) <br />site coordinator, classroom space for instruction/homework <br />assistance, playground/field and multi-purpose room for recreation <br />and enrichment activities <br />o Principal/CS Coordinator meet regularly (a minimum of monthly) with <br />site coordinator on program design, resources and problem-solving <br />
