<br />EXHIBIT "A" <br /> <br />SCOPE OF SERVICES <br />Independent Contractor Services provided <br />by Margaret Marshall for Redwood City 2020 <br /> <br />I. Provide coordination and administration for community dialogues about <br />"unnatural causes" of disease and lack of well-being during 2009. <br /> <br />Services include: <br />a) Working with RC 2020 staff and partners to identify potential dialogue groups <br /> <br />b) Establishing date and location for dialogues <br /> <br />c) Working with RC2020 and PCRC to determine most appropriate video segment <br />for use in the dialogue <br /> <br />d) Working with PCRC and JGC to schedule dialogue facilitators <br /> <br />e) Developing appropriate fact sheets for dialogue participants <br /> <br />f) Assuring materials and equipment for dialogues are provided <br /> <br />g) Collecting and collating notes reo discussion themes and action ideas that <br />emerge from dialogues <br /> <br />h) Overseeing participant evaluation of dialogue process <br /> <br />i) Working with RC2020 to move process from dialogue to action <br /> <br />Dialogue Coordination (estimate 10-15 hours/week) <br />Total compensation for Unnatural Causes Dialogue Project Mgt: <br />Source of funds: Catholic Healthcare West grant <br /> <br />$15,000 <br /> <br />II. Assist with grant proposal writing, monitoring and report writing. <br /> <br />a) At request of RC 2020 executive director, utilize grant proposal guidelines to <br />prepare proposals for funding to support RC 2020 priority areas. <br /> <br />b) Provide high quality, timely, grant-writing services. <br /> <br />c) Develop and coordinate schedule for grant reporting for grants provided to RC <br />2020. <br /> <br />d) Assist with grant report development as needed and requested. <br /> <br />Agreed upon rate for grant writing: $50/hour <br />Estimate of hours to be developed for each grant proposal to be written <br />Source of funds: Redwood City 2020 core budget <br /> <br />Agreement over $1 OK 10 <br />City Attorney Approved Version 111803 <br />
