(OSSMC) <br />FISCAL YEAR 2008-2009 <br /> <br />CONTRACT OBJECTIVES <br /> <br />Quantifiable Goals to be Provided With Performance Indicators (how you will measure <br />ReQuested Funds the Quantifiable goal) <br />We will increase the number of volunteers in South By increasing the base of volunteers we will improve <br />County by 2 new volunteers. our facility coverage by 5% over the coverage <br /> provided in FY 07/08 <br />We will investigate a minimum of 85 complaints Based on past history, we anticipate resolving 75% <br />involving unduplicated individuals in Redwood City of all cases to the satisfaction of the resident or the <br /> resident's responsible party. Every complaint <br /> resolved satisfactorily is indicative of an <br /> improvement in quality of life or quality of care for <br /> the resident(s). <br /> <br /># of Unduplicated RWC Residents/Households to be Assisted (Only programs serving individuals <br />ma report b individuals. All others should re ort b households) <br />In effect we will be providing assistance to all 880 residents in the long-term care facilities as our <br />ombudsman will monitor all 64 facilities durin the course of the rant eriod. <br />
