<br />. <br /> <br />EXHIBIT" A" <br /> <br />SCOPE OF SERVICES <br /> <br />Youth Leadership Institute <br />Project Management: Alcohol and Other Drug Prevention Planning Process <br />in Redwood City and North Fair Oaks <br />2008-09 <br /> <br />1. Establish structure and planning process to ensure community-based leadership in the <br />planning process <br />a) Establish steering committee of stakeholders to participate in prevention planning <br />process <br />b) Convene monthly steering committee of stakeholders <br />c) Develop agendas and meeting objectives in coordination with RWC 2020/JGC <br /> <br />2. Identify and implement assessment to elicit input and identify community needs around AOD <br />issues in RWC <br />a) Work with steering committee to collect existing community data that is relevant and <br />indicators that indicate AOD environments and use <br />b) Determine additional information to be gathered and methods <br />c) Develop material, resources, and tools for community convening OR other identified <br />assessment activities for broader stakeholder input <br />d) Support community convening (may not be appropriate) <br />e) Provide assessment tools and resources <br />f) Support steering committee review of data and information collected <br /> <br />3. Identify and prioritize community based prevention strategies <br />a) Provide technical assistance or training to steering committee on best practices in <br />environmental prevention strategies <br />b) Review current and existing prevention efforts, determine gaps according to need <br />c) Identify key areas of need and strategies <br /> <br />4. Develop data-driven implementation plan with key partners <br />a) Develop goals and objectives related to community need <br />b) Develop outcomes and indicators of progress <br />c) Determine roles and responsibilities of partners to participate in plan <br />d) Develop evaluation plan <br /> <br />Payments <br /> <br />YLI will submit to Redwood City 2020 a detailed invoice for payment in January 2009 (for one <br />half of the total amount ($11,000) and a final invoice for the second half of the total ($11,000) <br />when the project has been successfully completed. <br /> <br />Redwood City 2020 will submit the approved invoice to the City of Redwood City for payment <br />within 30 days of receipt of the invoice. <br /> <br />Agreement over $1 OK 10 <br />City Attorney Approved Version 111803 <br />
