In order for your organization to renew eligibility for <br />three (3) years, the enclosed Eligibility Renewal Application and any related form must be <br />completed. Non - renewal will cause your agency to be ineligible to receive any additional <br />surplus property <br />In the resolution section of the application, please provide the names of the five (5), or less, <br />individuals you are authorizing to obtain federal or state surplus personal property on behalf of <br />your organization, and include their titles and original signatures. Please understand that there <br />are no exceptions to this new regulation regarding the limitation of naming only five 151 key <br />individuals who will be recognized as your organization's primary current representatives. All <br />previous authorizations of personnel have been rescinded. <br />The resolution Section B of the application must be completed and signed by the governing <br />board or council secretary. A copy of the dated resolution page(s) or board /council minutes <br />verifying the board /council approval of the renewal and appointment of authorized <br />representatives is to accompany the renewal form. Section C remains blank. <br />In addition to completing the Eligibility Renewal Application, please have your board or council <br />secretary read and sign the enclosed federal form, "CERTIFICATION REGARDING <br />DEBARMENT, SUSPENSION, INELIGIBILITY AND VOLUNTARY EXCLUSION —LOWER <br />TIER TRANSACTIONS." Your agency is the donee. <br />Please submit the original completed application and federal government certification to my <br />attention at the above address as soon as possible.
