<br />During the months of March through October when most of the chemical applications and <br />harvesting is being performed~ Sara Burnsed will administrate bi-weekly tailgate safety <br />meetings. These meetings will stress workers safety ~ review of proper spray techniques, <br />equipment calibration, proper equipment maintenance requirements, driving safety and proper <br />docking of the harvesters at our new location under the bridge. It is through these bi-monthly <br />safety meetings that we at Waterworks Industries can make its best efforts to ensure that all laws, <br />regulation and safety procedures are being followed. <br /> <br />During the months of November through February, the surveys should be limited to twice <br />monthly as the observation are geared more toward winter time storm runoff of nutrients, <br />pollutants, debris and oil and grease discharges. The survey reports for months are reported the <br />same as the reports during the March through October time period. <br /> <br />The surveys will be billed at the rate included in our Fee schedule. As Waterworks Industries <br />has a yard in Redwood City, all billing time for travel is determined from the Redwood City <br />yard. Waterworks will anticipate the needed chemicals for the applications to be made for the <br />coming month and purchase the chemicals in advance so we are not waiting on deliveries. In <br />Exhibit C of this Proposal, please see the example of a survey report. <br /> <br />C) REPORTING TO CITY AND REGULATORY AGENCIES <br /> <br />As per the Redwood Shores Lagoon Management Plan (June 2003) a report will be submitted to <br />the City detailing the upcoming vegetation management season. In the report should be a <br />summarization of the time frames for anticipated work based on a review of work schedules <br />from previous year(s) and survey results thus far for the year. A comparison of the weather <br />conditions should also be noted. This type of information is necessary to have in the report to <br />properly anticipate the impending vegetation season. This report will be due on March 15, 2009 <br />and will be compiled and written by Rich Carnation and Sara Burnsed, with input from <br />Waterworks Industries staff. <br /> <br />At the end of each survey, Waterworks Industries will make a verbal report to City of Redwood <br />Cities designated staff. Waterworks Industries will prepare Form, A which summarizes the <br />lagoon surveys. Additionally, if chemical applications or harvesting were needed, Form B <br />summarizes harvesting and/or Form C summarizes chemical applications. Each form will be <br />filled out and sent to Redwood City by the 10th day of the following month detailing all the <br />services performed the prior month. Each form should correspond with the billing charges for <br />that month. <br /> <br />Additionally, Waterworks Industries shall submit the Department of Pesticide Regulations <br />required chemical use report detailing the chemicals that were used during the previous month. <br />That report is due by the 10th day of the following month after the chemical use. The report is <br />sent directly to San Mateo County Agriculture Commissioners office. A copy of this report will <br />be included in the monthly billing and corresponding forms A, B, and C. <br /> <br />Per the requirements of the Redwood Shores Lagoon Management Plan (June 2003) a report will <br />be submitted to the City detailing the events of the year as it pertains to the work that was <br />performed during that year.
