ORIGINAL <br /> <br /> AGREEMENT <br /> <br /> for <br /> <br /> SENIOR CASE MANAGEMENT AND OUTREACH <br /> <br /> (FAMILY SERVICE AGENCY) <br /> <br /> THIS AGREEMENT, made and entered into this ~,.~ day of <br />, 2001, by and between the CITY OF REDWOOD CITY, a <br />municipal corporation of the State of Califorrfia, (City), FAMILY SERVICE <br />AGENCY., a California nonprofit corporation (Contractor); <br /> <br /> WITNESSETH: <br /> <br /> WHEREAS, Contractor conducts senior case management and outreach for <br />persons who are residents in the City (the Program); and <br /> <br /> WI-IF, REAS, Contractor has requested financial assistance in order to conduct <br />the Program for residents of City during the fiscal year con~nencing July 1, 2001, and <br />ending June 30, 2002; and <br /> <br /> WHEREAS, City has reviewed said request and desires to allocate to Contractor <br />the sum of Fifteen Thousand and no/100 Dollars ($15,000.00) from its Community <br />Development Block Grant funds for the Program; <br /> <br /> NOW, THEREFORE, the patties hereto agree as follows: <br /> <br /> 1. Financial Assistance. City shall allocate to Contractor the sum of Fifteen <br /> Thousand and no/100 Dollars ($15.000.00) for exclusive use by Contractor during the <br /> fiscal year con~mencing July 1, 2001, and ending June 30, 2002, solely for the purposes <br /> described in paragraph 2. <br /> <br /> 8/15/01 <br /> <br /> <br />
