EXHIBIT "A" SCOPE OF SERVICES AND FEE Services to be Performed Consultant will ensure Program does the following: Program will engage in a rapid, multi -phase survey study designed to identify the highest priority needs from households in the community. Surveys will be of the entire household, and also anonymous, digital, 5-10 minutes in length, and include validated questions of unmet basic needs, knowledge and uptake of local resources, and demographic information. The studies will utilize two broad recruitment channels: surveys of individuals already seeking support services (food donations, rental assistance, health services) and surveys distributed through large city-wide stakeholders (school districts, city office, Sequoia Healthcare District). Surveys will occur serially, with the first to be released in late 2020 and later surveys to be distributed in 2021-2022. Each survey will incorporate stakeholder feedback from previous cycles, include questions pertaining to emergent issues of the time, and hold certain key questions constant to track the ebb and flow of need in the community. The Gardner Center will brief the City of Redwood City on findings from each survey and integrate feedback and suggestions from said briefings in subsequent surveys.
