EXHIBIT "A" <br />SCOPE OF SERVICES AND FEE <br />Services to be Performed <br />Consultant will ensure Program does the following: <br />Program will engage in a rapid, multi -phase survey study designed to identify the highest <br />priority needs from households in the community. Surveys will be of the entire household, <br />and also anonymous, digital, 5-10 minutes in length, and include validated questions of <br />unmet basic needs, knowledge and uptake of local resources, and demographic <br />information. The studies will utilize two broad recruitment channels: surveys of individuals <br />already seeking support services (food donations, rental assistance, health services) and <br />surveys distributed through large city-wide stakeholders (school districts, city office, <br />Sequoia Healthcare District). Surveys will occur serially, with the first to be released in late <br />2020 and later surveys to be distributed in 2021-2022. Each survey will incorporate <br />stakeholder feedback from previous cycles, include questions pertaining to emergent <br />issues of the time, and hold certain key questions constant to track the ebb and flow of <br />need in the community.
