REV: 11-22-2021 SK <br />Exhibit “A” <br />SCOPE OF SERVICES AND FEE <br />Subrecipient will provide services subject to the following revisions to the activity and program <br />budget: <br />1) The maximum allowable grant to each childcare provider is increased from $10,000 to <br />$20,000. <br />2) The amount of assistance that can be provided to each childcare provider is increased <br />from one month to up to 6 months. <br />3) The maximum amount of funds that can be spent on indirect and administrative costs is <br />increased from $25,000 to the maximum allowable amount of $27,500. <br />4) A budget line item was added for direct application technical assistance to clients. <br />The following is the revised activity and program budget: <br />Budget Line Item Revised Amount Notes <br />Provider Stipends $198,100 Max allowable grant to each <br />childcare provider is $20,000 <br />and assistance can be <br />received for up to 6 months. <br />Emergency Supplies $20,000 <br />Direct Application Technical <br />Assistance to Clients <br />$29,400 <br />Indirect & Administrative Cost <br />(10%) <br />$27,500 <br />Total $275,000 <br />Program Objectives and Qualitative Goals <br />The COVID-19 Child Care Provider Assistance program is expected to assist 15 child care <br />providers with this funding. <br />Subrecipient must comply with the reporting requirements, as well as document the analysis <br />conducted to prevent the duplication of benefits to any child care providers assisted, as specified <br />in HUD Notice August 7, 2020, FR 6218-N-01. <br />ATTY/AGR.2021 - Amend No. 1/Child Card Coordinating Council of San Mateo County (Page 3 of 3)
