EXHIBIT "A" <br />SCOPE OF SERVICES AND FEE <br />Scope of Services <br />Contractor shall provide window treatment supplies and installation services to the City <br />for Fire Station 10 as described below. <br />• Dorm Rooms <br />o Contractor shall provide and install eight (8) roller shades with cassette top <br />and stainless steel chain manual lift loops. <br />o The shade fabric shall be Essential Taupe (a room -darkening cloth). <br />o Contractor shall install one (1) shade for pairs of windows. <br />Kitchen Area, Gym, Bathrooms and Stairway Landing <br />o Contractor shall provide and install fifteen (15) roller shades with cassette <br />top and stainless steel chain manual lift loops. <br />o The shade fabric shall be Plexus 1 % Provisions (a light filtering cloth). <br />o Contractor shall install one (1) shade for pairs of windows in the gym and <br />kitchen. <br />o Contractor shall install two (2) shades on the sliding glass door. <br />o In the upstairs bathroom, Contractor shall install one (1) shade on the <br />window near the shower and one (1) shade on the window near the toilet. <br />o In the downstairs bathroom, Contractor shall install one (1) shade inside the <br />window frame. <br />Payment Terms and Fee <br />Contractor shall only submit an invoice once all services and supplies described above <br />have been provided and installed. <br />The parties agree that the price for materials and services are as follows: <br />Item <br />Price <br />Materials 23 Shades <br />$11,498.00 <br />Sales Tax <br />$1,121.06 <br />Installation Services <br />$2,070.00 <br />Total <br />$14,689.06 <br />REV: 05-04-21 MI <br />ATTY/AGR.2021.097/Broadway Cleaners (Window Treatment Supplies & Installation) (Page 11 of 11) <br />
