REV: 06-24-21 MI <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />I. SCOPE OF SERVICES <br />Hazardous Materials Remediation Observation <br />1.Consultant will perform daily observation and monitoring of the removal of the <br />hazardous materials impacted by the construction activities. At the completion of <br />abatement activity, Consultant will conduct a final visual of the abatement areas to <br />assure that the contractor removed these materials in accordance with the project <br />documents. It is estimated 25 days (shifts) will be necessary for this work. Work <br />will occur on an 8 hour per day basis. <br />2.Consultant will perform air monitoring utilizing Phase Contrast Microcopy (PCM) <br />for asbestos. Consultant will take up to 4 PCM samples/shift. All asbestos area <br />samples will be analyzed on-site by our NIOSH 582 trained and PAT proficient <br />technicians or an accredited laboratory. <br />3.Consultant will also perform daily air monitoring for lead, as needed. Lead air <br />samples will be analyzed by Flame Atomic Absorption (FLAA). Up to one lead air <br />sample will be collected daily as needed and submitted to an accredited laboratory <br />with a 24 hour turnaround. <br />4.Consultant will record daily activity on field forms that inform the owner of what <br />occurred on the site as it pertains to the abatement. Upon completion of the <br />abatement activity, Consultant will provide the owner or the owner’s representative <br />with a clearance memo for the phase of the project that has been completed. <br />5.Deliverables include a closeout report. The report will include field observation <br />forms, analytical results and any other pertinent documentation and will be <br />provided in electronic (PDF) format. <br />Consultant takes no responsibility for the contractor’s required personnel air sampling that <br />is mandated by the governing regulatory agencies, nor any contractor deliverables set <br />forth in the contract documents. This report will be generated at the end of the project. <br />FEES <br />Hazardous Materials Remediation Observation <br />Consultant will perform these services on a time and materials basis not to exceed <br />$27,500.00. This not to exceed amount is based on an estimated 25 shifts of abatement <br />oversight (price per shift is $1,100.00) during normal working hours (Monday to Friday 6 <br />am to 6 pm) for 8-hour shifts. This fee includes technician time for one technician, project <br />management, clerical, equipment, analytical fees for daily monitoring and, and report. <br />Actual oversight needs will be determined by contract schedule and can change, as <br />needed.
