REV: 08-29-2022 SK Exhibit “A” SCOPE OF SERVICES AND FEE Contract Term The PLHA Rapid Re‐housing Program will be extended through October 31, 2023. The existing scope of services and performance metrics will not change. Once the clients’ rental subsidy ends, the Program Administrator will continue to follow up on a regular basis with weekly or bi‐weekly calls for up to six months, as circumstances warrant. If this service is provided after October 31, 2023, Program Administrator will continue to provide this service without charge to then‐current clients. LifeMoves will invoice the City based on an expense based compensation model. For example: •Case Manager works XX hours- invoice bills for XX hours •LifeMoves pays YY deposits and ZZ monthly subsidies (Client Assistance and Flex Funds) – invoice bills for YY deposits and ZZ monthly subsidies. Hourly rates: •Case Manager: $30.87/Hour Personnel Program Director – supervision will be provided without allocation to this project. Case Manager – additional allocation for labor to cover extended contract term. Fringe & Benefits – additional allocation to cover additional labor Indirect Cost Additional allocation to cover operating expenses over a longer period of time.
