REV: 08-25-21 RL <br />“ ”EXHIBIT A <br />SCOPE OF SERVICES <br />Funding for this agreement has been granted to support the COVID-19 vaccine outreach efforts. <br />Siena Youth Center will play a lead role in coordinating activities between August – November <br />to help increase knowledge around the vaccine. <br />Consultant will: <br />1. Increase COVID-19 vaccination rates among Redwood City and North Fair Oaks by <br />increasing community health center capacity to provide patients with vaccines. <br />a. Insure that there is bilingual staff at vaccination and outreach events to help assist if <br />needed. <br />2. Help reduce mistrust or vaccine hesitancy within their target population. <br />a. Conduct vaccination outreach through existing messaging channels (daily text <br />reminder system, bulletin boards, social media). <br />3.
