Through this Order Schedule, Customer, as set forth above, desires to purchase a 1Life <br />Enterprise Subscription, including 1Life membership (“Membership”) for Eligible Customer Participants and any Expanded Offerings, as selected <br />below. Membership provides the Member the benefit of services made available from time to time to Members in jurisdictions where One <br />Medical providers are licensed. Customers receive launch, engagement, and open enrollment support and will receive enrollment and utilization <br />reporting. <br />Membership Subscription <br />[Core-Plus] <br />Eligible Customer Participants includes: <br />Each participant of the Customer as specified below (each, an “Eligible Individual” or “EI”). Metropolitan areas in which One Medical clinics are located are <br />referred to below as “Markets.” Available Markets as of the Effective Date are listed below. Eligible Individuals located outside of Markets are referred to as <br />“Virtual EI’s” and their Memberships are “Virtual Memberships.” Members in Markets have physical and digital One Medical access.
