REV: 11-29-2021 RL <br />EXHIBIT A <br />CITY OF REDWOOD CITY <br />Fresh Lifelines for Youth <br />FISCAL YEAR 2021-2022 <br />Agency Information <br />Agency Name:Fresh Lifelines for Youth (FLY) <br />Address:330 Twin Dolphin Drive, Suite 105 <br />City, State, Zip:Redwood City, CA 94065 <br />Activity/Program Title:FLY Law and Leadership Programs for Redwood City Youth <br />Quantitative Goals <br />Description of Services to be Provided: <br /> FLY will provide its Law and Leadership Program to 24 at-risk and system-involved Redwood City <br />youth. Youth ages 14-21 who are at risk of continued or future involvement in the juvenile system, <br />gain tools to lead self-sufficient and productive lives. The Law Program is an innovative law-related <br />education course taught in 2-hour sessions over 12 weeks by volunteer law and university <br />students, community professionals, and FLY staff. The Law Program uses discussions of youth’s <br />rights and responsibilities under the law as a starting point for developing their social-emotional <br />learning (SEL) skills, such as self-awareness, social awareness, critical thinking, and self- <br />advocacy. Youth who need more support are invited to join the Leadership Program, which <br />provides 10 months of case management and systems navigation support. In the Leadership <br />Program, case managers help youth stabilize their basic needs, work toward educational goals, <br />and gain skills for adulthood. Youth also participate in group positive social events and community <br />service activities together, which help them develop community supports and leadership skills. <br />Primary Quantitative Objectives: <br />FLY will serve 24 incorporated Redwood City persons in 24 households. <br /> <br />NOTE: The grant performance period begins July 1, 2021 and concludes June 30, 2022 <br />ATTY/AGR.2021.306/Fresh Lifelines for Youth (Page 10 of 10)
