Funding <br />from HSFA will support the core nutritional and wellness services for older adults age 60+ and over <br />at the Fair Oaks Adult Activity Center in Redwood City. These meals are provided as grab and go <br />meals. Since COVID-19 Fair Oaks Adult Activity Center is closed to in-person services, the model <br />has been adjusted to support the long term impact of the health and wellness of vulnerable, <br />primarily low-income older adults. <br />Primary Quantitative Objectives: <br />Peninsula Family Service will serve 220 incorporated Redwood City persons in 220 households. <br /> <br />NOTE: The grant performance period begins July 1, 2021 and concludes June 30, 2022 <br />ATTY/AGR.2021.294/Peninsula Family Service (HSFA FY 21-22) (Page 10 of 10)
