REV: 11-16-2021 RL <br />FUNDING AGREEMENT FOR HUMAN SERVICES <br />FINANCIAL ASSISTANCE PROGRAM <br />BETWEEN THE CITY OF REDWOOD CITY AND <br />SAMARITAN HOUSE <br />THIS AGREEMENT, made and entered into this day <br />of , 2021, by and between the CITY OF REDWOOD CITY, a charter city and <br />municipal corporation of the State of California (“City”), and Samaritan House, a California <br />nonprofit corporation (“Contractor”). Collectively, the City and Contractor will be referred <br />to as the “Parties.” <br />RECITALS <br />A. WHEREAS, Contractor provides a free clinic that addresses health inequity <br />for underserved, low-income residents of southern San Mateo County(“Program”); and <br />B. WHEREAS, Contractor has requested financial assistance in order to <br />conduct the Program for residents of City during the fiscal year commencing July 1, 2021, <br />and ending June 30, 2022; and <br />C. WHEREAS, City has reviewed the request and desires to allocate to <br />Contractor the sum of Ten Thousand Dollars ($10,000). <br />NOW, THEREFORE, the Parties hereby agree as follows: <br />1. FINANCIAL ASSISTANCE. City shall allocate to Contractor the sum of Ten <br />Thousand Dollars ($10,000) for exclusive use by Contractor during the fiscal year <br />commencing July 1, 2021, and ending June 30, 2022, to be used by Contractor solely for <br />the purposes described in Section 2. <br />2. USE OF FUNDS. Contractor shall use the funds provided pursuant to <br />Section 1 solely for the Program purposes described in Exhibit “A” (Contract Objectives) <br />attached hereto and by this reference incorporated herein. <br />3. CHANGES TO PROGRAM. No changes in the Program as described in <br />Exhibit “A” to this agreement shall be made without the prior written consent of City. <br />4. PAYMENTS. Contractor will keep detailed and accurate records of all <br />expenditures made and expenses incurred which are funded under this agreement. <br />Payments of funds allocated pursuant to Section 1 shall be made to Contractor within 30 <br />calendar days after Funding Agreement is fully signed and executed. <br />5. STANDARD OF SERVICE. Contractor warrants to the City that the <br />Program will be performed to the highest standards provided for projects of a similar or <br />like nature in the San Francisco Bay Area, as determined by City’s Human Services <br />Manager. Without limiting the generality of Contractor’s obligations pursuant to Section <br />14, Contractor will be responsible for, and hold City harmless from, any failure to adhere <br />to such standards.
