REV: 02-01-2022 RL <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />Contractor shall provide maintenance services for the preservation and protection of <br />Heating Ventilating, and Air Conditioning (HVAC) equipment and systems at City-owned <br />facilities. Services shall be provided at the request of the Public Works Services Facilities <br />Superintendent or their designee. <br />Services may include: <br />•Routine preventative maintenance <br />•Diagnostic troubleshooting <br />•System testing <br />•System inspections <br />Contractor shall respond within 2hrs for same day call-outs. <br />Fee: <br />City shall pay Contractor on a time and materials basis per the fee schedule below in an <br />amount not to exceed One Hundred Thousand Dollars ($100,000). Materials purchased <br />by Contractor to complete required services shall be billed with a 32% mark-up. Invoices <br />shall be billed upon completion of services with Net 30 payment terms, in a form <br />consistent with Section 6, Compensation. <br />HVAC Maintenance Technician $190/hour <br />ATTY/AGR.2022.017/Bayside Heating & Air Conditioning, Inc. dba CP Mechanical (Page 11 of 11)
