Construction Services Agreement <br />REV: 04-26-2021 PR <br />00 5205 - 6 <br /> <br /> <br />Appendix A to Construction Services Agreement <br />SCOPE OF WORK <br /> <br />Contractor shall provide the City with the following services at the Fair Oaks Community Center located at 2600 <br />Middlefield Road, Redwood City CA 94063. All work shall be completed in accordance with site drawings provided <br />by the City. <br /> <br />Demolition of existing office workspaces: <br /> Demolition and removal of existing infrastructure (Per Appendix G) <br />Restroom: <br /> Demolish and fill existing doorway (Per Appendix G) <br /> Cut and install new doorway and frame (Per Appendix G) <br />New Construction: <br /> Construct and frame all new offices per Appendix G (Dark gray lines represent new walls, solid white lines <br />represent existing walls) <br /> <br />o Framing <br />o Insulation <br />o Drywall <br />o Mud/Tape/Smooth Wall <br />o Paint <br /> Furnish and install all doors and sidelight including framing and door hardware (per Appendix G) <br /> <br /> All finish work to be completed per Appendix G <br /> <br /> Intake Counter per drawing A3.0 Section 7 shall be Quartz, color to be determined by City. <br /> <br />Fee: <br /> <br />City shall pay contractor on a per task basis per the fee schedule below in an amount not to exceed Three <br />Hundred Fifty Thousand Dollars ($295,999). <br /> <br /> <br /> Task Description Amount <br />1 Demo existing offices, <br />designated walls, doors and <br />fixtures <br />Flat Rate Amount of $24,500 <br />2 Construct new offices per <br />design from the Crosby Group <br />(Appendix G) <br />Flat Rate Amount of $271,499 <br /> Total Not to Exceed $295,999
