REV: 02-08-22 RL <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />Scope of Services <br />Contractor shall provide services for the supply and installation of Cured-In-Place (CIPP) <br />lining and related services as outlined below for a 15” Strom Drain located on Buckeye <br />Street. <br />•Mobilization and demobilization <br />•Clean and TV 15" Storm Drain <br />-Pre-video inspections and documentation of existing pipe <br />-Bypass pumping mainline flows up to 250 GPM <br />•CIPP Line 15" Storm Drain <br />-Installation of CIPP lining per specifications including inversion, curing <br />and finishing. Contractor to utilize a polyester resin system. <br />-Internal later reinstatement <br />-Post video inspection following completion of the installation <br />The Contractor shall follow confined space safe entry practices and provide a one-year <br />warranty. <br />Schedule of Work <br />Work to commence the week of March 14, 2022 <br />Work to be completed by April 4, 2022 <br />Fee schedule <br />Consultant shall receive a flat fee of $25,000 upon completion of all mobilization, pipeline <br />cleaning and TV, and cured-in-place pipe lining serves. Additional services shall be billed <br />on an hourly basis not to exceed $7,500. The applicable hourly rate is $450/hour.
