EXHIBIT 3 <br />STATEMENT OF WORK <br />Project Name- <br />Installation Services <br />Seller Representative: <br />Tim Sharkey <br />847-419-7355 <br />timshar@cdwg.com <br />Customer Name: <br />Redwood City (CA) <br />CDW Affiliate: <br />CDW Government LLC <br />SOW Created Date: <br />October 10, 2020 <br />Solution Architect: <br />Saba Mirza <br />Version: <br />3 <br />This statement of work ("Statement of Work" or "SOW") is made and entered into on the date signed by both parties (the <br />"SOW Effective Date") by and between the undersigned, CDW Government LLC ("Provider", "Seller" and `eve") and <br />Redwood City (CA) ("Customer" and "you'). <br />PROJECT DESCRIPTION <br />PROJECT SCOPE <br />SUMMARY <br />Provider to assist Customer with Basic Managed Installation services for approximately 142 systems across two (2) locations <br />in listed in attached Exhibit. <br />INSTALLATION SERVICES: <br />Schedule technician for onsite Professional deployment, including but not limited to: <br />1. Unpack and validate equipment received, as well as condition of equipment. Discrepancies and damages will be <br />reported to Provider Project Manager (PM) <br />2. Validate equipment against packing slip <br />3. Move device from central, onsite location to installation location <br />4. Connect unit to pre-existing AC power supply or UPS and neatly dress the cable <br />5. Attach the cables that are Customer provided from keyboard, mouse, monitor, docking station and network and <br />neatly dress them, <br />6. Basic connection to existing network via existing network jack and patch cable or wireless router <br />7. Check basic system functionality; workstation is clean; discrepancies reported. <br />8. Customer must have all necessary passwords available <br />9. Verify existing internet connectivity via web browser through a hard data connection to a switch/router and wireless <br />network connection if available <br />10. Test and register each device with Dell or Lenovo support. <br />11. Download and install Microsoft windows updates <br />12. Once the installation is completed, remove packing materials to Customer designated location at the site or reuse for <br />De -installation of existing equipment <br />13. Provide Customer with an inventory list of completed computer host names and serial numbers. <br />DE -INSTALLATION SERVICES: <br />1. De -install Desktops & Laptops <br />2.
