REV: 04-21-2022 RL <br />EXHIBIT “A” <br />SCOPE OF SERVICES <br />Consultant will: <br />Deliverable <br />Description <br />Action Item Due Date <br />Increase COVID-19 <br />testing rates among <br />Redwood City and <br />North Fair Oaks by <br />increasing community <br />health center capacity <br />to provide patients <br />with testing. <br />Insure that there is <br />bilingual staff at <br />testing sites and <br />outreach events to <br />help assist if needed. <br />June 2022 <br />Help reduce mistrust <br />or vaccine hesitancy <br />within their target <br />population. <br />Conduct testing <br />outreach with <br />flyers/information <br />received from <br />Redwood City <br />Together through <br />existing messaging <br />channels bi-weekly to <br />reach at least 100 <br />community members <br />(daily text reminder <br />system, bulletin <br />boards, social media). <br />June 2022 <br />Educate our most <br />vulnerable <br />populations on <br />concerning issues in <br />the community. <br />Host educational <br />events <br />(physical/mental <br />health focus etc…) for <br />community members <br />once a month with <br />programmatic space. <br />April, May and June <br />2022 <br />Consultant will be paid in one installment upon completion of all services and pursuant <br />to an invoice consistent with Section 6, Compensation. <br />ATTY/AGR.2022.094/Redwood City PAL (Page 10 of 10)
