REV: 04-25-2022 RL Exhibit “A” A. SCOPE OF PURCHASE AND FEE I. Scope of Purchase Vendor shall provide one (1) submersible tank mixing system intended for continuous use while submersed in a potable water storage tank with chloraminated water, specifically a PWM400V3 PAX Water Mixer with a PCC405V3 PAX Control Center submersible mixing system with controls and accessories necessary for a complete and operable system. Vendor shall not be required to provide the following items: •Equipment unloading and installation •All civil works and concrete pad for equipment •Any underground or structural work •Anchor bolts and seismic restraints •Heat tracing and insulation of all interconnecting equipment •Room ventilation, air conditioning or lighting •Any video recording •Electrical power to control panel •Any electrical conduit runs •Any tank recoating services, labor, or parts •Any tank hatch penetrations •All electrical conduit, wiring, electrical material, etc. between control panel, SCADA, etc. •All taxes, fees, lien waivers, bonds and licenses •Any permitting or regulatory approvals •Any items not mentioned under this agreement Vendor shall deliver the 1) NSF Certification (1 Electronic Copy) and 2) Installation, Operations, and Maintenance Manuals (1 Hard Copy and 1 Electronic Copy) (collectively, “Submittals”) within two weeks after the Water Public Works Superintendent or designee authorizes the purchase through a purchase order. Vendor shall then ship the equipment to the Public Works Services Administration Building within two weeks after Vendor receives written notice that the Water Public Works Superintendent or designee has approved the Submittals. All physical items shall be delivered to the Public Works Services Administration Building, located at 1400 Broadway, Redwood City, CA 94063. B. INSURANCE 1. Insurance. Vendor will obtain and maintain for the duration of the Agreement and any and all amendments, insurance against claims for injuries to persons or damage to property, which may arise out of or in connection with delivery of the Goods or performance of the Services by Vendor or Vendor’s agents, representatives, employees ATTY/AGR.2022.097/PAX Water Technologies, Inc.
