REV: 05-17-2022 RL <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />Contractor shall provide disposal services of mixed concrete and asphalt containing no <br />dirt at a rate of $200 per load at the facility located at 100 Graniterock Way, San Jose, <br />CA 95136, at the direction of the Public Works Services Director or their designee. <br /> <br />•The facility must be able to accept and dispose of 20 loads minimum daily during <br />Contractor’s business hours. <br />•The current City of San Jose sales tax will apply and be added to all charges for <br />disposal services requested by the Public Works Services Director or their <br />designee. <br />•The City will load out materials from an end dump truck with 20 tons maximum per <br />load. <br />•The City does not guarantee, nor agree, to purchase a minimum quantity of <br />disposal services. <br />Contractor shall be paid on Net 30 terms upon receipt of detailed monthly invoices that <br />comply with Section 6 above. <br />Invoices must be sent to: <br />Redwood City Public Works <br />1400 Broadway <br />Redwood City, CA 94063 <br />ATTY/AGR.2022.111/Granite Rock Company (Page 11 of 11)
